Par le sperme et les sécrétions vaginalesdonc par les relations sexuelles sans préservatif avec pénétration anale ou vaginale, qui sont le mode majeur de transmission du virus, autant pour les hommes que pour les femmes. Le risque d’infection par le VIH augmente statistiquement avec le nombre de partenaires, mais un seul rapport non protégé avec une personne atteinte peut suffire pour être contaminé. Quant aux contacts de la bouche avec le sexe ou l’anus, les quelques cas de contamination de ce type sont extrêmement rares. Cependant, il convient d’éviter de recevoir du sperme dans la bouche.Par le sangdonc par le partage des seringues chez les toxicomanes qui doivent aussi se protéger de la contamination par voie sexuelle en utilisant des préservatifs.Par transmission materno-foetaleil existe un risque de transmission de la mère séropositive à son enfant, durant la grossesse ou pendant l'accouchement.
